I have received permission from my doctor to participate in this program (if applicable).
I understand and accept that neither the instructor, nor the studio, is liable for any injury, or damages, to person or property, resulting from taking the class. I release my photo and video footage to be used for promotional and educational purposes. Those under 18 years of age must have this form signed by a parent or guardian.
I understand and accept that the administrator reserves the right to remove any participant from the group for any reason deemed necessary without notice and without refund. This typically only applies if one is causing intentional emotional distress or disruption to the group.
